ABSTRACT

Objective: to investigate the spread of knowledge regarding silicosis and to identify the occupational health nurse course of action to prevent it. Method: It is an integrative review of the literature held in the databases: LILACS, SciELO, BDENF and PubMed. The employed keywords were “silicosis”, “occupational health nursing” and “pneumoconiosis”, published in Portuguese, Spanish and English, in the years of 2004 to 2014. Eight articles met the inclusion criteria were selected and answered the guiding questions. Results:The selected articles seek to deepen the knowledge on silicosis, to estimate its prevalence, to establish the patient’s profile, to describe the major symptoms, to identify the major risk factors associated with the disease and to evaluate the quality of living of those afflicted by it. Conclusion: Silicosis is a public health problem with high prevalence and which requires very specific strategies to its prevention and control, especially those developed in occupational health nursing.


Objetivo: averiguar a divulgação do conhecimento sobre silicose e identificar ações do enfermeiro do trabalho para sua prevenção. Métodos: Trata-se de uma revisão integrativa da literatura realizada nas bases dedados LILACS, SciELO, BDENF e PubMed, utilizando-se os descritores “silicose”, “enfermagem do trabalho” e “pneumoconiose”, nos idiomas português, inglês e espanhol, dos anos de 2004 a 2014. Foram selecionados oito artigos que atenderam aos critérios de inclusão e responderam as questões norteadoras. Resultados: Os artigos selecionados abordam a silicose, estimam sua prevalência, delineiam o perfil dos portadores, descrevem os principais sintomas, identificam fatores de risco associados e avaliam a qualidade de vida dos acometidos. Conclusão: A silicose é um problema de saúde pública com alta prevalência, que necessita de estratégias específicas para seu controle e prevenção, especialmente aquelas desenvolvidas no âmbito da enfermagem do trabalho.

Método: Se trata de una revisión integradora de la literatura realizada en las bases de dados LILACS, SciELO, BDENF y PubMed utilizando las palabras clave “silicosis”, “enfermería de trabajo” y “neumoconiosis”, en Portugués, Inglés y Español. Fueron seleccionados de los años 2004 a 2014 ocho artículos que cumplieron con los criterios de inclusión y respondieron a las preguntas de orientación. Resultados: Los artículos seleccionados cubren silicosis, valorar su prevalencia, definir el perfil de los pacientes, describir los síntomas principales, identificar los factores de riesgo asociados y evaluar la calidad de vida de los afectados. Conclusión: La silicosis es un problema de salud pública con una alta prevalencia, que requiere estrategias específicas para su control y prevención, especialmente las desarrolladas en la enfermería de trabajo.

Contexto: Silicose é uma doença pulmonar ocupacional crônica, fibrogênica, de evolução lenta, caráter progressivo e irreversível, causada pela exposição à sílica livre. Esclerose sistêmica (ES) é uma doença de caráter autoimune, com alterações vasculares e fibrose tecidual difusa. Associação entre ambas, denominada síndrome de Erasmus, já é conhecida há vários anos e geralmente ocorre após 15 anos do início da exposição à sílica. Objetivo: Revisão da literatura referente à associação entre silicose e ES. Método: Selecionados artigos nacionais e internacionais obtidos na íntegra, referentes à pneumoconiose, silicose, ES e a associação entre essas duas últimas doenças, sendo incluídos aos resultados os manuscritos que abrangiam especificamente a síndrome de Erasmus. Resultados: Encontrados na literatura nacional relatos de casos de síndrome de Erasmus, os quais apresentavam ES e exposição prévia à sílica com duração que variou de 2 a 15 anos, atividade laboral interrompida há mais de 7 anos, pacientes do sexo masculino, média de idade 46 anos, sendo a principal atividade jateamento de areia; e havia relato em 3 casos de uso inapropriado de equipamentos de proteção. Entre os cinco casos em que a tuberculose foi investigada, três apresentaram a associação entre tuberculose pulmonar e síndrome de Erasmus. Conclusão: Importância do reconhecimento precoce da associação entre silicose e ES, dando ênfase na história ocupacional. O diagnóstico da síndrome de Erasmus por pneumologistas, reumatologistas e médicos do trabalho permitirá intervenções mais efetivas e um tratamento precoce, que proporcionarão um melhor prognóstico para esses pacientes.


Context: Silicosis is a chronic occupational lung disease, fibrogenic, slowly evolving, progressive and irreversible, caused by the exposure to silica. Systemic sclerosis (SSc) is an autoimmune disease with vascular changes and diffuse tissue fibrosis. The association between both, called Erasmus syndrome, has been known for several years and usually occurs after 15 years from the beginning of exposure to silica. Objective: To review the literature regarding the association between silicosis and SSc, known as Erasmus syndrome. Method: Selected national and international articles retrieved in full, related to pneumoconiosis, silicosis, SSc and the association between these two last diseases; manuscripts which specifically dealt Erasmus syndrome were included in the results. Results: Reports of Erasmus syndrome were found in the national literature, which presented SSc and previous exposure to silica with duration ranging from 2 to 15 years of exposure, interrupted employment business for over 7 years, male patients, mean age of 46 years, being the main activity sandblasting, and 3 cases of improper use of protective equipment were reported. Among the five cases in which tuberculosis was investigated, three showed the association between pulmonary tuberculosis and Erasmus syndrome. Conclusion: Importance of early recognition of the association between silicosis and SSc, with emphasis on occupational history. The diagnosis of Erasmus syndrome by pulmonologists, rheumatologists and occupational physicians will enable more effective interventions and early treatment, which will provide a better prognosis for these patients.

Introdução: atualmente, entre as doenças ocupacionais incapacitantes, 60% são atribuídas ao sistema respiratório, sendo 85% dos casos relacionados à silicose. Objetivo: este estudo avalia os efeitos respiratórios da inalação ocupacional de sílica em trabalhadores de marmorarias. Métodos: foram incluídos 21 trabalhadores de marmorarias da cidadede Gurupi-TO, no período de março a maio de 2012. Foram submetidos ao questionário de sintomas respiratórios e hábito tabágico. Todos realizaram espirometria. Para comparação intergrupo dos fumantes, ex-fumantes e não fumantes foi usado o teste paramétrico ANOVA e para outras análises o teste não paramétrico Fisher e adotado o nível de significância de 5% de probabilidade. Resultados: a média da idade dos trabalhadores foi de27 ± 6 anos. A prevalência de indivíduos sintomáticos correspondeu a 57%. Ao comparar os grupos quanto à presença ou não de sintomas, relação ao tempo de exposição à sílica (> ou < 5 anos), hábito tabágico, IMC e às variáveis espirométricas obtidas, verificou-se ausência de significância estatística. Conclusões: não foi possível identificar associação entre os sintomas detectados e a exposição à sílica, o que não exclui sua provável interação e reforça a necessidade de mais estudos específicos, tais como espirometria seriada associada à análise de volumes pulmonares e testes de aptidão cardiorrespiratória comergoespirometria para avaliação mais ampla e direcionada nesses indivíduos.


Introduction: Among the currently disabling occupational diseases 60% are attributed to the respiratory system, with 85% of cases related to silicosis. Objective: This study assesses the respiratory effects of occupational silica inhalation among marble industry workers. Methods: 21 workers employed in the marble industry in the town if Gurupi-TO between March and May 2012 answered a questionnaire about their respiratorysymptoms and smoking habits. All underwent spirometry. For intergroup comparison of smokers, ex-smokers, and nonsmokers the parametric ANOVA test was used and for other analyzes the nonparametric Fisher test, with a significance level of 5% probability. Results: The average age of workers was 27±6 years. The prevalence of symptomatic patients was 57%. Comparing the two groups for the presence or absence of symptomsby length of exposure to silica ( > or < than 5 years), smoking status, BMI, and spirometric variables obtained, there was no statistical significance. Conclusions: no association between the detected symptoms and exposure to silica could be identified. This does not exclude its probable interaction and reinforces the need for more specific studies, such asserial spirometry associated with lung volume analysis or cardiorespiratory fitness tests and exercise tests for a broader assessment targeted at these individuals.

Para dar respuesta a la pregunta ¿Cómo es la historia natural de la silicosis en una población de trabajadores mineros de Perú?, se aplicó el análisis de supervivencia de Kaplan - Meier a la data de 4056 mineros peruanos que laboraron entre 1955 y 2006, y que buscaron voluntariamente asistencia médica entre los años 2003 y 2006. Se encontró casos de Silicosis aguda, acelerada y crónica, que progresa rápidamente una vez instalada. - Toma 4.7 años pasar de Silicosis 0 a Silicosis III -disminuyendo la esperanza de vida en todos los casos. El periodo de latencia desde el inicio de la exposición laboral hasta el diagnóstico de Silicosis 0 fue de 20.11 años, pero para trabajadores mineros de subsuelo este periodo de latencia fue de 13.45 años, y para trabajadores de minería de superficie fue de 21.07 años. La probabilidad de sobrevivir sin diagnóstico de silicosis los primeros 5 años de vida laboral fue de 0.9871+/-0.004. La mediana de supervivencia global al diagnóstico de silicosis fue de 25.7 años. La probabilidad de sobrevivir sin diagnóstico de silicosis luego de 50 años de exposición laboral, aun cuando la exposición haya cesado fue de cero. Se demostró que en la historia natural de la silicosis influyen factores como edad, número de empresas en que laboró el huésped, área de superficie corporal, peso, índice de masa corporal, presión arterial, tipo de minería donde laboró el huésped (superficie, subsuelo)...


To answer the question: What is the natural history of silicosis in a population of miners?, we applied survival analysis Kaplan-Meier dating from 4056 to the Peruvian miners who worked between 1955 and 2006, and voluntarily sought medical care between 2003 and 2006. We found cases of acute silicosis, accelerated and chronic, which progresses rapidly once installed, takes 4.7 years to move from Silicosis 0 to Silicosis III - declining life expectancy in all cases. The latency period from the start of occupational exposure to the diagnosis of Silicosis 0 is 20.11 years, but for underground mine workers this latency period is 13.45 years and for surface mining workers is 21.07 years. The probability of surviving without silicosis diagnosis of the first 5 years of employment is 0.9871+/-0.0004. The median overall survival from diagnosis of silicosis is 25.7 years. The probability of surviving without diagnosis of silicosis after 50 years of occupational exposure, even when exposure has ceased is zero. It was shown that in the natural history of silicosis factors influenced by factors such as age, number of companies that worked the host, body surface area, weight, body mass index, blood pressure, type of mining where he worked the host (surface, subsurface)...

ABSTRACT

Silicosis had been considered for decades as an illness with manifestations of lung fibrosis due to inhalation of overconcentrated SiO2 dust. To the best of our knowledge, studies have yet to report SiO2 deposits in any other tissues and organs. In the present case, while performing bilateral artificial total hip arthroplasty for one patient, we found that the articular cartilage of the bilateral femoral head was black. Therefore, specimens thereof were sent for pathological examination. Pathological examination (immunohistochemistry) and polarized light microscopy revealed the presence of considerable brown, acicular, rhombic, and crumb-like crystals. The crystals were mainly composed of SiO2. SiO2 could deposit in vessels and femoral head cartilage via blood circulation.

ABSTRACT

A silicose é a pneumoconiose mais frequente, decorrente da inalação de sílica ou de poeiras minerais contendo silicatos, caracterizada principalmente pela fibrose pulmonar de caráter irreversível. Está associada com o desenvolvimento de outras doenças, incluindo tuberculose pulmonar, câncer pulmonar e doenças autoimunes. A doença do tecido conjuntivo posterior à exposição à sílica ocorre geralmente após 15 anos do início da exposição. A síndrome de Erasmus refere-se ao desenvolvimento de esclerose sistêmica em indivíduos previamente expostos à sílica e que apresentavam ou não silicose. Os autores relatam dois casos de pacientes com diagnóstico de silicose que desenvolveram esclerose sistêmica.


The silicosis is the pneumoconiosis more frequent, resulting from the inhalation of silica or silicates containing mineral dust, mainly characterized by irreversible lung fi brosis. It is associated with the development of other diseases, including pulmonary tuberculosis, lung cancer and autoimmune diseases. The connective tissue disease after exposure to silica occurs usually after 15 years of initial exposure.The Erasmus syndrome describes the association of systemic sclerosis following exposure to silica with or without silicosis. The authors report two cases of patients with diagnosis of silicosis who developed systemic sclerosis.

La silicosis es una neumoconiosis producida por inhalación repetida de polvo de sílice, caracterizada por fibrosis pulmonar y acompañada de problemas bronquíticos. Es factor predisponente para cáncer pulmonar y determina incapacidades laborales temporales o permanentes. De acuerdo a su forma de presentación, la silicosis puede ser clásica, aguda, complicada o acelerada. El Perú es un país minero con más de 197 mil trabajadores directos en esta actividad considerada de riesgo alto para contraer silicosis. El examen clínico, los antecedentes laborales, la radiografía de pulmones y la espirometría son la base del diagnóstico. Para la silicosis no existe tratamiento, por lo que la prevención es la única forma conocida de evitarla. La principal medida preventiva es la educación del trabajador y del empleador para hacerles conocer el riesgo de exposición a polvo de sílice y las medidas de protección colectiva y personal


Silicosis is a pneumoconiosis caused by repeated inhalation of silica dust, characterized by pulmonary fibrosis and a bronchitis component. It is predisposing factor for lung cancer and cause of temporary or permanent work disability. According to its presentation it may be classical, acute, complicated or accelerated. Peru is a mining country with more than 197 000 direct workers in this activity considered at high risk for developing silicosis. Clinical examination, labor history, lung X-rays and spirometry are basic in diagnosis. There is no treatment for silicosis. Prevention is the only known way to avoid it. In addition to collective and personal protection, main preventive action is education of workers and employers on risk of exposure to silica dust

ABSTRACT

A 70-yr-old woman visited our hospital for shortness of breath. Chest CT showed ground glass opacity and traction bronchiectasis at right middle, lower lobe and left lingular division. Video-assisted thoracic surgical biopsy at right lower lobe and pathologic examination revealed mixed dust pneumoconiosis. Polarized optical microscopy showed lung lesions were consisted of silica and carbon materials. She was a housewife and never been exposed to silica dusts occupationally. She has taken freshwater snails as a health-promoting food for 40 yr and ground shell powder was piled up on her backyard where she spent day-time. Energy dispersive X-ray spectroscopy of snail shell and scanning electron microscopy with energy dispersive x-ray spectroscopy of lung lesion revealed that silica occupies important portion. Herein, we report the first known case of silicosis due to chronic inhalation of shell powder of freshwater snail.

ABSTRACT

As pneumopatias ocupacionais tem se constituído num tópico importante no contexto das políticas nacionais onde se observa gradualmente uma implementação nas diretrizes da saúde pública em relação à saúde do trabalhador. O presente artigo tem como objetivo apresentar uma revisão literária sobre o assunto, destacando as formas clínicas mais prevalentes no Brasil e buscando correlacionar os aspectos preventivos e médico-legais envolvidos nas pneumoconioses.


The occupational pneumopathy has become an important topic in the contex of national politics where we can see a gradual implementation of public health guidelines in relation to the worker health. The present article has the objective to present a literary revision about the issue, highlighting the most common clinical forms in Brazil, trying to correlate the preventive and legal medicine aspects involving pneumoconiosis.

Existe escassez de estudos brasileiros analisando o efeito da exposição contínua à sílica em ex-trabalhadores de minas de cobre. Os achados radiográficos produzidos pela silicose têm recebido atenção médica especial. O objetivo deste estudo foi caracterizar clinica e radiologicamente a silicose em ex-mineiros de extração de cobre. Foi feito um estudo transversal com 100 ex-mineiros de extração de cobre. Os itens foram coletados através de entrevista pessoal, dados clínicos ocupacionais e foram realizados exames radiológicos. Na entrevista foi verificada a idade, o sexo e o tempo de trabalho na extração de cobre. Os pacientes que já realizaram exame radiológico de tórax trouxeram estes para comparação. Os mineiros assinaram um termo de consentimento para realização dos exames radiológicos o qual foi aplicado pelo radiologista que os interpretou. Entre os 100 ex-mineiros, 35 por cento são portadores de silicose, 11 por cento apresentaram alterações pulmonares que podem ter outras causas, como tuberculose, enfisema e doença broncopulmonar obstrutiva crônica, e 54 por cento não apresentaram nenhuma alteração pulmonar. A alta freqüência de silicose encontrada reforça a recomendação de suspender a exposição à sílica tão logo se tenha uma radiografia sugestiva da doença.


There is a dearth of Brazilian studies analyzing the effect of ongoing exposure to silica among former copper mine workers, as radiographic readings for silicosis have received widespread medical attention. The scope of this study is to investigate the clinical and radiological manifestations of silicosis among former copper mine workers. A sample of 100 former copper miners had their clinical and occupational histories recorded and underwent chest radiography in a cross-sectional study that included personal interviews in which their age, sex and time worked in copper extraction were recorded. Patients with recent chest X-Rays brought them with them, or they signed a term of consent and X-Rays were taken and analyzed by the radiologist using the Student t Test and Pearson test. Among the 100 miners, 35 percent had silicosis, 11 percent had conditions other than silicosis, such as tuberculosis, emphysema, chronic pulmonary obstructive disease, and 54 percent had no pulmonary lesions. The high incidence of silicosis reinforces the recommendation that exposure should be halted as soon as an X-ray detects the condition.

ABSTRACT

Se realizó un estudio cualitativo para conocer las experiencias de lareubicación laboral en trabajadores con silicosis a través de describir,tematizar e interpretar las vivencias, en el periodo de octubre a diciembre de 2009 en una empresa minera en Chile. A través de tres estudios de caso, donde el criterio de inclusión más importante fue el diagnóstico de silicosis categoría 1. Se aplicaron entrevistas en profundidad basadas en una guía de entrevista. El análisis se enfocó enlos cuatro existenciales fenomenológicos. En los resultados se encontró que los trabajadores describieron de forma profunda sus relaciones familiares, sociales y laborales (“de mi familia recibí todo el apoyo en cuanto se supo de mi enfermedad”). Percibieron el apoyo de su jefatura y de sus compañeros de trabajo. Sobre la reubicación laboralmanifestaron: “no es solo que te cambien de lugar para trabajar, sinodonde tú puedas aportar para el desarrollo de la empresa y donde túpuedas desarrollarte profesionalmente”. Mostraron conciencia de su enfermedad, refiriéndose a una buena cobertura de salud otorgada por la Unidad de Salud Ocupacional de Clínica Río Blanco S.A. Esimportante continuar explorando otros casos de reubicación laboral por silicosis para comprender con mayor profundidad el fenómeno.


We conducted a qualitative study to learn from the experiences of jobrelocation in workers with silicosis by describing, the ming and interpret the experiences, from October to December 2009 at a mining company in Chile.There were 3 case studies where the most important inclusion criterion was the diagnosis of silicosis category 1. In-depth interviews were applied on the basis of an interview guide. The analysis focused on the four existential phenomenological. The results found that a profound workers described their family relationships, social and labor "My family got as much support as soon as he learned of my disease." Perceived support from their leadership and their colleagues. Expressed on job relocation "is not just that they will change fromplace to work, but where you can make to the development of the company and where you can develop professionally. They showed awareness of his illness, referring to good health coverage issued by the Occupational Health Unit Clínica Río Blanco SA It is important tocontinue exploring other job relocation cases of silicosis to understand more deeply the phenomenon.

ABSTRACT

Silica activates release of biochemical substances in lungs. To evaluate duration dependent toxic effects of silica by biochemical changes in Broncho Alveolar Lavage Fluid (BALF) and by post-mortem findings we developed a series of rat silicosis. Based on duration of exposures, single intratracheal injection of quartz dust in saline to acute models (n=3) and inhalation of quartz dust with air to chronic models (n=3) were administered. Control rats received vehicle only. Group wise BALF was collected on completion of exposure periods. Post-mortem examination was performed. Protein, hydroxyproline, elastase and Elastase Inhibitory Capacity (EIC) in BALF were measured. Post-mortem examination revealed progressive fibro-nodular changes in lungs. Biochemical parameters excepting EIC in both models showed significant (p< 0.001) gradual rise. Duration dependent biochemical changes in BALF due to silica were found responsible for progressive morbidity and may be considered as early markers for diagnosis, thereby, preventing increasing morbidity and death.

ABSTRACT

A silicose é uma doença crônica e incapacitante com altas taxas de mortalidade e morbidade tornando-se uma preocupação não só no território nacional mas a n´vel internacional. A silicose é a pneumoconiose mais antiga e mesmo assim, medidas preventivas e tecnologia disponível nas indústrias não são suficientes para diminuir sua incidência. Mesmo com desenvolvimento de sistemas legislativos mais eficientes e conscientes, surtos de silicose aguda continuam a ocorrer em indústrias mesmo nos países mais desenvolvidos. Uma vez estabelecida a doença é irreversível tendo como única alternativa evitar a progressão da doença e o surgimento de complicações . Este trabalho foi realizado através de uma extensa revisão da literatura nacional e internacional, utilizando livros texto e artigos publicados nas principais revistas médicas com o objetivo de conhecer melhor a doença e sua epidemiologia, diferentes apresentações e possíveis formas de manejo terapêutico.

A silicose, a mais prevalente das pneumoconioses, é provocada pela inalação de partículas de sílica cristalina. Indivíduos expostos à sílica, com ou sem silicose, apresentam risco aumentado de tuberculose e de micobacterioses não-tuberculosas. O risco de silicóticos desenvolverem tuberculose em relação a controles sadios varia de 2,8 a 39 vezes, em conformidade com a gravidade da doença de base. Têm sido estudados diferentes esquemas de quimioprofilaxia para tuberculose em silicóticos, todos com eficácia semelhante e com redução final de risco para cerca da metade em relação ao uso de placebo. São, no entanto, esquemas de longa duração, o que, acrescido dos possíveis efeitos colaterais (particularmente hepatotoxicidade), podem prejudicar a aderência. As diretrizes atuais recomendam a realização de prova tuberculínica e, se positiva, a instituição de quimioprofilaxia. São vários os esquemas possíveis, tanto em termos de drogas quanto de duração. Nossa recomendação é de que se use isoniazida na dose de 300 mg/dia (ou 10 mg/kg/dia) por seis meses para os indivíduos com silicose ou sadios com exposição superior a 10 anos, se forem reatores fortes à prova tuberculínica (induração > 10 mm). São necessários, no entanto, novos estudos para que indicações, drogas, doses e duração da profilaxia sejam definidas mais apropriadamente.


Silicosis, the most prevalent of the pneumoconioses, is caused by inhalation of crystalline silica particles. Silica-exposed workers, with or without silicosis, are at increased risk for tuberculosis and nontuberculous mycobacteria-related diseases. The risk of a patient with silicosis developing tuberculosis is higher (2.8 to 39 times higher, depending on the severity of the silicosis) than that found for healthy controls. Various regimens for tuberculosis chemoprophylaxis in patients with silicosis have been studied, all of which present similar efficacy and overall risk reduction to about one half of that obtained with placebo. Long-term regimens have potential side effects (particularly hepatotoxicity). In addition, the use of such regimens can jeopardize adherence to treatment. The current guidelines recommend that tuberculin skin tests be performed, and, if positive, that chemoprophylaxis be instituted. There are several possible regimens, varying in terms of the drugs prescribed, as well as in terms of treatment duration. We recommend the use of isoniazid at 300 mg/day (or 10 mg/kg/day) for six months for patients with silicosis, as well as for healthy patients with periods of exposure to silica longer than 10 years and strongly positive tuberculin skin test results (induration > 10 mm). Nevertheless, further studies are necessary so that indications, drugs, doses and duration of chemoprophylaxis regimens can be more properly defined.

Para conhecer a prevalência de silicose, descrever os perfis clínico, epidemiológico e ocupacional, e estudar as condições de trabalho das oficinas de lapidação, realizou-se estudo transversal de setenta lapidários de pedras semipreciosas de Joaquim Felício, Minas Gerais, Brasil, de abril a dezembro de 2002. Utilizaram-se história ocupacional com questionário respiratório, radiografia de tórax e espirometria. A prevalência de silicose foi de 7,1 por cento. Todos eram do sexo masculino, com média de idade de 21,5 anos e tempo médio de exposição à sílica de 7,1 anos. Todos pertenciam ao setor da economia informal e trabalhavam em oficinas pequenas e rudimentares, sendo o maquinário improvisado e havendo maciça exposição à sílica. Segundo a Classificação Radiológica da Organização Internacional do Trabalho, dos cinco casos de silicose, quatro foram classificados na categoria 1, e um na 3. A silicose em lapidários constitui um sério problema de saúde pública que requer esforços para minimização do risco de adoecimento, por meio da ação de equipes multidisciplinares provenientes de instituições governamentais e não-governamentais, com inserção ativa dos trabalhadores.


A cross-sectional study was performed to determine the prevalence of silicosis among semiprecious gem cutters (n = 70) and describe their clinical, epidemiological, and occupational profiles in Joaquim Felício, Minas Gerais State, Brazil, from April to December 2002. Occupational history was used with a respiratory questionnaire, chest x-ray, and spirometry. Silicosis prevalence was 7.1 percent. All affected individuals were men, with a mean age of 21.5 and mean exposure to silica of 7.1 years. All belonged to the informal economy and had been working at small and rudimentary workshops, using improvised lathes, with intense exposure to silica dust. According to the ILO Radiological Classification, four of the five cases of silicosis were classified as grade 1 and one as grade 3. Silicosis among gem cutters constitutes a serious public health problem that requires efforts to minimize risk through work by multidisciplinary teams from government agencies and nongovernmental organizations, with active participation by the workers themselves.
